Making a claim regarding a strain
The boxes marked in bold type on this form are compulsory. In the event of claims made as a result of being sent the wrong cultures or for broken freeze-dried samples it is not necessary to fill in the boxes related to the condition of the culture. Remember that you should indicate a valid telephone number and an e-mail address so that we can get in touch with you.
Claims must be made within thirty days of delivery, one this period is over we do not accept any claims made for strains.
